Either print the registration forms off below or come into the surgery to pick up the registration forms.
Once the forms have been filled in please bring them to the surgery along with ID and a receptionist will look through the forms to see if everything has been filled in correctly, do not post them into the surgery as if they have not been filled in correctly it will delay your registration. If you are bringing in a registration for a child please bring in their Personal Child Health Record (red book) so we can take a copy of their immunisations this is to make sure they do not miss any scheduled immunisations while we wait for their records to come over to the surgery.
Once we have accepted the registration it can take up to 2 weeks to register you at the surgery.
