You can register with us by collecting registration forms from the surgery, downloading them from our website, or requesting them by email.
Once you have completed the forms, please bring them to the surgery in person, along with photo ID if you have it.
A member of our reception team will check that the forms have been completed correctly before accepting them. This helps avoid delays with your registration.
Please do not post your registration forms through the surgery letterbox, as we may not be able to process them if information is missing.
Registering a Child
If you are registering a child, please bring their Personal Child Health Record (red book) if you have it. This allows us to take a copy of their immunisation record so they do not miss any routine vaccinations while we wait for their medical records to arrive.
Health Check
To help us keep your medical records up to date, we ask all adult patients to use our height, weight and blood pressure machine when registering.
The machine will print a results slip. Please hand this slip to reception along with your registration forms.
Using the machine is not mandatory, but we would appreciate it if you are able to do so, as it helps us record your current health measurements when you join the practice.
How Long Does Registration Take?
Once we have accepted your registration forms, it can take up to 2 weeks for your registration to be completed.
If you need urgent medical care before your registration is complete, please contact the surgery. We will do our best to process your registration as quickly as possible so you can receive the care you need.
