Data Protection form
For the surgery to release copies of your medical records you need to fill in a Data protection form and return it to the surgery along with the relevant id for this, please see form below.
If you cannot print this form you can get one from the surgery.
This process can take up to 28 days and will be emailed to you using the email address given on the form you filled in. If you don't have access to an email account then they can be printed but this will need to be specified on the form.
